Survey of Nurse Aide Registries (Direct Care Worker) in the United States

The survey results highlight inconsistencies and gaps in training, testing, and other pre-employment requirements for nursing assistants. The results from the nurse aide registries are presented in a series of charts. The survey notes who pays for pre-certification training in each state, whether candidates are allowed to become certified by passing the test without undergoing training, whether nurse aides pay a fee to be licensed or certified, who does criminal background checks, and more.

Accountability Focus Series: Performance Measurement and Outcome Reports

Colorado’s interest in performance measures reflects the recognition that the first step in improving service system performance is to measure it. As part of its implementation of the Systems Change Project, Developmental Disabilities Services has tracked system performance against specified “key” performance indicators. The Accountability Focus Series includes: Key Indicators of Performance; Outcomes Of Services and Supports. The third report is Gauging How Well Colorado Supports Its Citizens.

Real Choice Systems Change Grants: Compendium Fourth Edition

The Compendium contains basic information about each of the Real Choice Systems Change Grantees. The Compendium will help you learn more about how these grants will be used to allow more people of all ages with a disability or long term illness to live and participate in their communities. Real Choice Systems Change Grantees will also find the Compendium useful to identify other Grantees with similar goals and activities. The fourth edition of the Compendium replaces all previous editions.

Handbook for Transition Navigators

This handbook, drafted by Jane Schiele and edited and revised by Laura Hershey (HCPF Systems Change Section), is designed to help other advocates or agencies to inform many more individuals about their right to live in the community, and to assist those who desire a transition back to their community. The report defines transition and offers the nuts and bolts of the program and lastly overs tips for moving day and beyond.

After Leaving the Nursing Home Form

This survey was developed by the Colorado Facility Transition grant to evaluate the transition process of individuals across Colorado. Results from this survey were used to develop and implement changes in the transition process to better serve the population. The form is for people who have moved out of a nursing home in Colorado within the last two years. How you fill in this form is important because the answers help other people who will move from a nursing home soon.

The State of the States in Family Caregiver Support: A 50-State Study

This report focuses attention on a growing issue facing the nation: how to support & sustain those who are the cornerstone of our LTC system—the dedicated families & informal caregivers. This national study is the first to examine publicly funded caregiver support programs throughout all 50 states. Fall 2004 National MIG Conference - National Consortium for Health Systems Development

Promising Practices in Long Term Care Systems Reform: Common Factors of Systems Change

Medstat identified common factors of system change in a six-page brief, based on a review of organization transformation literature and, more importantly, the experiences of eight state long-term support systems that were the subject of a series of case studies on comprehensive system reform.

Technical Assistance to Real Choice Consumer Task Forces-Website

The Centers on Medicare and Medicaid Services awarded a three year contract to the Topeka Independent Living Resource Center (TILRC) to provide technical assistance to the Consumer Task Forces Real Choice system change grant projects. In doing this, CMS highlighted the contributions that consumers can and must make to the success of the grants and to increasing access to community living services more generally. This website offers a list of grant projects, tip sheets and survey tools.
