TAE Issue Brief: Strategies for Building Collaboration

ADRC grants are infrastructure grants aimed at creating new or formalizing existing relationships among state and local agencies and public partners; collaboration among these entities is critical for ADRC development. This briefing paper will offer ideas and strategies for building a collaborative environment and building effective partnerships.

Short-Term Case Management for Aging and Disability Resource Centers (ADRCs): National Vision and the Wisconsin Experience- A Fact Sheet

This fact sheet offers guidance on Short-Term Case Management (STCM) using both the vision offered by AoA and CMS and the experience in Wisconsin which has operated an ADRC for six years. STCM is an integral part of an ADRC - to offer consumers information about and access to long-term supports and services in order to minimize confusion, enhance individual choice, and support informed decision-making. This fact sheet describes STCM as a philosophy of relationship development with individuals.

TAE Issue Brief: The Medicare Modernization Act and ADRCs

This issue brief describes the implications of the Medicare Prescription Drug, Improvement and Modernization Act of 2003 for consumers, state Medicaid agencies, health care providers, and the public and private entities that provide benefits counseling and supports to Medicare and Medicaid beneficiaries. The Medicare Modernization Act (MMA) made a number of significant changes to law that affects ADRCs, most notably, MMA established prescription drug coverage for Medicare beneficiaries.

Users may be unable to use a website’s navigational menu, fill out online forms, or even access content. Addressing accessibility issues is important. Throughout the long-term care system, there exists an institutional bias for placing people in institutional settings over HCBS settings. Aging and Disability Resource Center Guidance for Evaluation Plan

This document provides information on elements to include in the ADRC evaluation plan. This resource will provide guidance on what to include, and will serve as an example of how to organize the elements into a matrix, but this format can be tailored by each grantee. Elements to incorporate into the ADRC evaluation plan include: Project Objectives; Tasks; Indicators; Data Sources; Data Collection Methods; Timeline/Interval; Responsible Entity; and Instruments.

Issue Brief on Prevention: Considering Prevention in the Development of an Aging and Disability Resource Center

This article profiles some suggests on prevention which occurs on three levels. Primary prevention targets healthy individuals and groups to stop disease before it starts; secondary prevention targets those at early stages of disease or with high risk behaviors but who are asymptomatic; and tertiary prevention targets those with symptomatic disease, to manage or improve their conditions. ADRCs can promote health by offering information assistance and resources on each of these levels.

The ADRC program calls for aging networks to broaden its target population & closely coordinate with the disability network to establish seamless systems for consumers. This brief provides background information on characteristics of individuals with physical disabilities, describes the major information & service needs & the existing supports & services system. The brief also addresses implications for grantees in serving this population & provides additional resources to facilitate operations.
