This report focuses on individuals with Alzheimer's and other dementias. The project instituted a comprehensive system to support enduring changes in the delivery of community-based assistance for Medicaid eligible persons with dementia. The projects two goals: facilitate hospital-to-home transitions for individuals wishing to stay in the community; and field test a nursing home-to-community transition program for eligible individuals. The workbook used in discharge planner training is included.

This handbook documents the components of a successful information and assistance system. It is intended for use as a guide for service providers to develop or enhance their own I & A processes or to replicate the system utilized at Eldersource. Review their protocol for confidentiality, emergency calls, communications, reporting, plus sample job descriptions and resource list.

The booklet is a publication of the Louisiana Nursing Facility Transition Grant. It is a Self Assessment Transition guide to assist individuals in nursing facilities on how to start planning for a move to the community and obtaining the supports and services they will need. It includes contact information for available state programs, along with contacts for housing options.

Housing Within Reach is a project of the Tennessee Department of Mental Health and Developmental Disabilities. The project included a consumer driven longitudinal evaluation study across the state to document consumer perspectives on consumer housing preferences, and the availability, access and barriers to appropriate housing. The study included face-to-face interviews conducted with each study participant at baseline, and at six- and twelve-months.

The purpose of this report is to provide recommended actions steps for the state of Nevada to establish structural, programmatic and funding mechanisms to facilitate a balance in resources for people with disabilities moving from institutions to community living. The primary principle of Money Follows the Person is simple and straightforward - people with disabilities should be able to decide where and how their services and supports should be provided.

The Guardian Mentor Program was set up, in part, to assist legal guardians with issues, concerns and questions that may arise in the first year after someone from a State Center transitions to the community. This brochure explains the guardian-to-guardian mentor program which including support, information and advocacy for legal guardians.

This chart, developed by the North Carolina MIG, projects the estimated costs and enrollment in health coverage for individuals with disabilities in the workforce. The numbers are based on a cost of $1,210 per individual, per month and covers the five years of the program (2007-2012).

Employment for All, a regional initiative of the MIG project, the Granite State Employment Project, produced a slide presentation explaining their role in developing a cross-disability employment system in New Hampshire.

The New Hampshire MIG program, the Granite State Employment Project, and its programs within, Employment for All and the Monadnock Center for Successful Transitions, have held several planning meetings so far in 2007. Included below are the minutes for each of these meetings.

The Monadnock Center for Successful Transitions issued consent forms to eligible consumers to complete in order to participate in the Granite State Employment Project Consumer Forum. The purpose of the forum is to study issues regarding employment for individuals with disabilities in the Monadnock Region. The information gathered will be used in the strategic planning process for the Granite State Employment Project, the New Hampshire MIG. Also included is a list of the focus group questions.
