Louisiana\'s Plan For Immediate Action: Providing Long-Term Care Choices For the Elderly and People with Disabilities

This plan describes fundamental changes that states have implemented and found effective in controlling the public costs of long-term care while insuring that people receive quality services of the type, and in the settings, that they choose.

Self-Direction Orientation: Presentation

The New Opportunities Waiver (NOW) is the service delivery option offered by the Office for Citizens with Developmental Disabilities. Rhode Island’s Social Security Work Incentive Programs 1619 (a) and 1619 (b) Issue Brief

The Rhode Island MIG project produced an issue brief, which outlined the State’s utilization of 1619(a) and 1619(b) waivers, their effect on disabled workers, the barriers these waivers present, and recommendations for eliminating barriers and other best practices.

Pennsylvania Employment Resource Guide for Persons with Disabilities

The Pennsylvania MIG project developed the Work Incentive Planning Assistance (WIPA) Program in order to promote employment among those receiving SSI/SSDI benefits. WIPA produced the following newsletter entitled “Employment Resource Guide for Persons with Disabilities”, which sought to succinctly list services available to individuals with disabilities who wish to work. Some of these resources include Family Savings Accounts and information about the Volunteer Income Tax Assistance Program.

Foundations in Behavioral Healthcare: Integrating approaches to planning effective interventions for adults with mental illness, substance use disorders and cognitive impairments living in public housing - A Training Curriculum

This trainings series was developed in the Fall of 2006 by DATA of Rhode Island. The goals are to provide a foundation of behavioral health care knowledge and skills for human service workers who work with adults with a range of disabilities, with an ultimate goal of human service workers helping to affect better outcomes for the persons they serve. Each of the 14 training seminar ranges in duration from 3-6 hours. Review each training curriculum module which contains the outline and slides.

Maryland Service Access Information Link (SAIL) Online Application

SAIL, the Service Access and Information Link is an example of a web-based screening and application tool that allows Maryland applicants to complete a series of questions to help them decide which social services benefits to start an application process for. Consumers can start an application online for more than 9 programs, complete a redetermination, report changes and print verification forms. Organizations can become community partners to help customers apply for benefits.

Evaluation of the Maine Aging and Disability Resource Center Project

This final evaluation report reviews the Maine ADRC project which began with three sites designed to demonstrate the \"no wrong door\" concept. Part I of this report describes the experience of, and recommendations from, the DASH Network in Bangor. Part II of the report focuses on the experience of the 3 local coalitions and their member organizations and highlights coalitions' community development activities and recommendations for future development.

Program Improvement Plans: Overview and Summary Report

In response to the CFSR, states must prepare a Program Improvement Plan (PIP) that addresses deficiencies and builds upon strengths. The reviews examine several aspects of child welfare agency performance including outcomes for children as well as systemic factors supporting agency services. This brief summarizes specific effort in Rhode Island to address deficiencies in the Quality Assurance (QA) Systemic factor documented in PIPs completed in response to CFSR reviews between 2001 and 2003.

Children’s Intensive Services (CIS) Final Year Report and Issue Briefs

This report summarizes performance of CIS during its first year of operation. The program is an acute hospital diversion program that provides community-based services for youth (age 0 to 21) at risk for hospitalization or out-of home placement. During the first 12-months of program operations, nine community-based mental health agencies were certified to provide CIS services. The evaluation examines service utilization patterns and major findings while updates are provided in the issue briefs.

Medication Administration Training DVD

This DVD is an education project designed by the North Carolina Real Choice Workforce Infrastructure Development grantee and it demonstrates and reinforces the various kinds of medication aide training. The DVD is part of a traditional training course but offers a visual review on proper procedure. The DVD reviews records, hygiene, supplies and provides 13 examples of the various ways medication can be administrated.
