Promising Practices in HCBS: Minnesota- Quality Improvement Based on the Values of People Who Receive Supports

A pilot project in southeastern Minnesota has developed a quality assurance approach that focuses on matters important to people receiving supports and their families and provides a means of quality improvement, as opposed to licensing minimum standards. This report is based on an independent evaluation of the project, which used extensive interviews, focus groups and reviews of written materials and project files. The report provides intervention, implementation, and impact information.

Promising Practices in HCBS: Illinois- Simplified Access for HCBS Waiver and Older Americans Act Services

This report describes how the State of Illinois simplifies access to home and community-based services for people with disabilities age 60 and older. Local Case Coordination Units coordinate eligibility determination and case management for Medicaid waiver services, state-funded home and community-based services, and Older Americans Act services. This report explains the implementation and impact of this program.

Promising Practices in HCBS: South Carolina- Improving Responsiveness of Service Managers to Persons\' Needs

This report describes how the State of South Carolina is addressing a problem of inconsistency between an individual’s needs and the service plans developed to meet identified needs. South Carolina added a feature to its automated Case Management System that reminds case managers of needs identified in the assessment that may potentially be included in the service plan. The report details the interventions, implications, and impact of this automated system.

2001 Grantee Update: No cost extension, mandatory web-based reporting

Transcripts of Audio Conferences hosted October 15 and 22, 2003 to offer 2001 Grantees an opportunity to dialogue with Steven Lutzky, Ph.D, CMS Division for Community Systems Improvement. CMS provided critical information about a no cost extension, mandatory reporting, and more. Topics for discussion included: 1) Working with the new CMS web-based reporting system; 2) Reassignment of CMS Project Officers to better address Grantee needs; 3) Timing and Processes to request no cost extensions.

Promising Practices in HCBS: New Jersey- Single Access Point for Information on All Services for Older People

Midwestern Regional Housing Forum Introduction & Notes

The Future Supply of Long Term Care Workers in relation to The Aging Baby Boom Generation

This report is a product of collaboration between HHS and DOL in response to the requests from the U.S. Congress. Staff at these Departments worked collaboratively to share information and data pertaining to direct care workers in long-term care settings and to develop a joint set of recommendations for the future. This report includes information on informal caregivers, direct care workers, and paraprofessional direct services workers.
