Improving PA Home and Community-Based Care - Training Initiative

A Southwestern Pennsylvania demonstration program has been initiated to promote relationship-centered care for elders and people living with disabilities in need of home and community-based services and supports. It involves a series of five three-day train-the-trainer programs based on the state model for personal care aides (PCAs). Included are the announcement and a fact sheet on direct-care workers in Pennsylvania.

Increasing Person-Centered Thinking: Improving the Quality of Person-Centered Planning

Are you looking for ways to improve the quality of person-centered plans? This manual was developed to help improve the facilitation process around PCP. Most of the information uses the foundation of Personal Futures Planning, and the training provides an overview of the fundamental concepts and principles underlying this approach to planning. The manual is intended to be used as a resource in training programs.

Overview of the Community First Choice Option

Find a chart that summarizes the Patient Protection and Affordable Care Act, Community First Choice Option. This legislation amends 1915 of the Social Security Act to allow States to provide “Community-Based Attendant Services and Supports”. Information regarding financing, eligibility, scope of services, system design, quality management, evaluation and stakeholder involvement are included.

Works For Me – Pennsylvania

How is Pennsylvania marketing its MIG program called "Works for Me"? In October 2009, the Works For Me website went live, as did their billboards, TV ads and a direct mail campaign. Attached is a link to the website for this program which offers an audio version of each web page, sample newsletters, brochure, and billboard artwork. These samples could be useful to other states starting a MIG program of their own and/or interested in creating cohesive marketing campaigns.

Transition Action Guide – Utah

Seamless collaboration with and for students with disabilities transitioning to employment, education/training, and adult life is vital to making transition programs successful. Utah created a Transition Action Guide for students with disabilities and agencies to accomplish this. A sample copy, revised in April of 2010, is attached. A link is also provided to a copy posted on Utah’s State Office of Education site, where it is updated periodically.

Diversity Training Curriculum - Florida

Diversity Training is key to providing an inclusive climate in the workplace for disabled workers, as well as for creating these opportunities in the first place. The attached presentations are part of Florida’s Agency for Persons with Disabilities training curriculum. Intended to highlight the positive impacts working has on the disabled and on the workplace, these presentations are for employers and could be useful to other MIG grantees as well.

Community Living Program Fact Sheet

AoA launched the CLP initiative in the fall of 2007 with a vision to help states create person-centered programs that reach people at risk of institutionalization before they enter a nursing home. Since 2007, approximately $36 million has been invested in 28 states to develop the CLP program. With the implementation of the 2009 CLP grants, there will be more than 120 CLP program sites nationally. This fact sheet includes the program’s background, AoA’s vision for CLP and a list of the grantees.

Changing Systems, Changing Lives - 2010 AOA, CMS, and VA Grantee National Meeting

View presentations and handouts associated with this meeting held in Feb. 2010 in Alexandria, VA. Objectives of the meeting included increasing grantees' knowledge of successful strategies to propel their ADRC, CLP, and VDHCBS programs towards fully functioning status, showcasing best practices, promoting peer-to-peer exchange, building workforce capacity and encouraging successful partnerships for achieving sustainable reform. The conference featured a variety of plenary and workshop sessions.
