Pennsylvania Community Choice ADRC Survey and Assessment Instrument

The Pennsylvania ADRC program has developed a number of measurement instruments including three surveys and an assessment instrument. This program is a joint effort between Pennsylvania’s Office of Health Care Reform as well as the Departments of Aging and Public Welfare. The products include a survey for eligible, ineligible and enrolled consumers and a community choice assessment instrument.

Memorandum Of Understanding (MOU) Regarding Family Care Eligibility Determination And Enrollment- Wisconsin

The MOU is developed among the ADRC in Richland County, the Economic Support Services Unit (ESS), the enrollment consultant for WI Family Care program, and the Care Management Organization (CMO). The MOU is intended to help clarify the specific roles and responsibilities, ensure that the participating units understand the key elements of coordination and communication, and to facilitate their working in partnership to achieve quality outcomes for program consumers and the general public.

TAE Issue Brief: ADRC Roles in Rebalancing Long Term Care Systems: Diversion Initiatives

Throughout the long-term care system, there exists an institutional bias for placing people in institutional settings over HCBS settings. Most states have some form of nursing home diversion program however, these programs face several challenges. This paper outlines many of the challenges, details strategies, and offers resources.

Redesigning Long-Term Care Systems Through Integrated Access And Services

This document compares 4 Area Agency on Aging (AAA) models on integrating access and services in CA, MA, OH, & ME. The programs each use AAAs as entryways to senior and LTC services. The sites differ in terms of the primary financing mechanisms and whether the effort was initiated at the state level and disseminated downward or at the local level and worked its way up. Indiana Automated Case Management Software RFP

The Indiana Department of Administration (IDOA) released an RFP for a statewide-automated case management system that included case management, overall tracking & control of cases, person centered planning to formulate & monitor individually centered plans of service, management & control of provider claims, direct linkage to all state systems, outcome-related information, waiting list management to track unmet demands by type of service and individual, & electronic billing to include Medicaid.

Indiana’s Waiver Services Unit Program Overview

The Indiana Waiver Services is a new unit created in Jan ‘04, to consolidate the management and operation of Indiana’s eight HCBS Waivers. Attached are documents that provide a program overview. The Services Unit Summary describes the program; the Priority Diversion Guidelines details the individual criteria for priority diversion; the Waiver Services Organizational Chart provides a pictorial overview; and the Hoosiers at Home Press Release announces the programs goals.

Aging and Disability Resource Center Guidance for Evaluation Plan

This document provides information on elements to include in the ADRC evaluation plan. This resource will provide guidance on what to include, and will serve as an example of how to organize the elements into a matrix, but this format can be tailored by each grantee. Elements to incorporate into the ADRC evaluation plan include: Project Objectives; Tasks; Indicators; Data Sources; Data Collection Methods; Timeline/Interval; Responsible Entity; and Instruments.
