Long-Term Support Options Counseling: Decision Support in Aging and Disability Resource Centers

This paper presents the perspective that options counseling is a pivotal function of Aging and Disability Resource Centers (ADRCs) and its essential elements are counseling and decision support. The diverse array of definitions currently in use runs the risk of diminishing the importance of options counseling, which is a key function of a comprehensive long-term care system.

A Survey of North Dakota Consumers of Continuum of Care Services

This report summarizes the research findings of a 2006 questionnaire disseminated to gather information about choice and access to continuum of care services. The survey gained information from consumers regarding services they are using, services needed, and barriers encountered. Data was gathered regarding how consumers learn about available services and suggestions to guide the development of a single point of entry (SPE) system, also called an Aging and Disability Resource Center (ADRC).

Real Choice Systems Change Grants: Compendium Sixth Edition

The Compendium contains basic information about each of the Real Choice Systems Change Grant awards in FY03–FY04 plus the Family to Family and Systems Transformation Grantees awarded in FY05 and FY06. The Compendium will help Grantees identify others with similar goals and activities. Learn more about how the grants will be used to allow more people of all ages with a disability or long term illness to live and participate in their communities. The sixth edition replaces all previous editions.

UCare: A Website for Utah Caregivers

The UCare website is a feature of the Utah Department of Human services and exists primarily to help caregivers and provide link to other resources. Resources are for the elderly, people with mental illness, for people with disabilities and their caregivers. Information is available on-line and through a statewide toll free number. Other features to note include provider report cards, guides, a glossary and the long-term care planning tool.

Minnesota’s Real Choice Systems Changes Grant Presentations: 2004

Homeownership for People with Disabilities Forms

The Illinois Homeownership Coalition for People with Disability (HCPD) has a comprehensive set of documents pertaining to its advocacy and assistance for homeownership. The organization’s goal is to assist people with disability in purchasing their homes, allowing them to maintain independence and living in their communities. The Aging and Disability Resource Center (ADRC) Demonstration Grant Initiative -Interim Outcomes Report

Forty-three states and territories have received three-year competitive grants since the ADRC program was launched. This report details findings at the overall ADRC level and the pilot level on the outcomes, accomplishments, and contributions of the ADRC program over the grant period, including more immediate results related to key consumer and program outcomes. It also documents lessons learned and program and policy implications at the pilot, state, and national level.

Creating Livable Communities

The report presents six strategies or policy levers, gleaned from previous research reports that can be implemented on the federal and local levels to promote collaboration and coordination and support livable community objectives. Examples were selected from a vast array of actions that can be taken. Each of the strategies is illustrated by actual promising practices at both the federal and state levels that can be adapted and replicated elsewhere.

Leadership Transition and Succession Planning

ADRCs can be affected by both external and internal changes in leadership. Organizations that plan for and carefully manage a leadership transition, however, can minimize disruption and even use the change to shift direction and strengthen organizational capacity. This issue brief offers suggestions for ways ADRCs can manage transition and position themselves to take advantage of opportunities to strengthen collaboration and advance the goals of the ADRC program.
