Aging and Disability Resource Center

This memorandum of understanding was signed as part of the Vermont ADRC project between the Vermont Department of Aging and Independent Living and the Department of Children and Families (Medicaid). The MOU is informative for streamlining access efforts. Other ADRCs designing programs will find this MOU a useful example.

This report describes the process evaluation findings related to ADRC implementation approaches in three expansion sites, presents an initial evaluation of the role and functioning of the ADRC Coalition, and describes the formation of the collaboration between the AAA and the Regional Offices of MHDDAD. A framework includes benchmarks for successful collaboration development, and program logic modeling is discussed as an evaluation framework to guide future process and outcome evaluations.

These training materials used by the Michigan Long Term Care Connection include a list of learning objectives, a topic checklist, and three evaluation surveys.

South Carolina ADRC compiled this how-to manual for establishing an Aging and Disability Resource Center. This presentation is used for education and awareness about Virginia's No Wrong Door system. It describes the system of seeking services before No Wrong Door and then explains the programs goals, functions and model for collaboration among service providers.

The SC ADRC project developed these two PowerPoint Presentations used to 1) educate and train adults in South Carolina about the importance of and strategies involved in planning for long-term care needs in the future and 2) train parents or relatives of children with disabilities about future planning for themselves and their children.

Grantees are required to establish measurable performance goals for their programs as well as indicators that enable them to track progress. Relating to consumer and client satisfaction with services, it is important that established goals assess visibility, trust, ease of access and responsiveness of program functions and staff. This summary contains findings of ADRCs assessments of consumer satisfaction relating to performance indicators and reported in the semi-annual report.

Diverse partnerships ensure that the needs and interests of all aging and developmental disability client groups are represented. The Georgia ADRC developed this resource to help organizations build partnerships between Aging and Developmental Disability groups. Partnerships often emerge naturally between individuals and organizations who share a common vision or concern. This resource provides a series of questions and points to consider when partnering organizations and networks.

Georgia created this guide on developmental disabilities, including the definition, criteria for prevention and screening, and a list of specific conditions that fall under the heading of developmental disabilities. Each diagnosis contains an overview plus available resources. Also included is a disability related acronyms list and glossary.
