CDHS oversees the state’s 64 county departments of social/human services, the state’s public mental health system, Colorado’s system of services for people with developmental disabilities, the state’s juvenile corrections system and all state and veterans’ nursing homes, through more than 5,000 employees and thousands of community-based service providers.

Medicare, the federal health insurance program, covers most people age 65 or older, some people under age 65 with disabilities, and people with end-stage renal disease. This report describes how the State of Illinois simplifies access to home and community-based services for people with disabilities age 60 and older. Local Case Coordination Units coordinate eligibility determination and case management for Medicaid waiver services, state-funded home and community-based services, and Older Americans Act services. The District of Columbia, Department of Health, Medical Assistance Administration is undergoing an effort to improve its ability to perform case and care management for the aging and disabled populations within the District. As part of this process, MAA is seeking to implement an information system that can support effective care management assessment and planning.

The District of Columbia is seeking to expand the availability of home and community-based services (HCBS) for the aged and adults with physical disabilities.The Disability and Aging Resource Center will represent a cost to the District in the immediate short-term, however the Center is expected to have cumulative savings by FY 2005.
