This handbook documents the components of a successful information and assistance system. It is intended for use as a guide for service providers to develop or enhance their own I & A processes or to replicate the system utilized at Eldersource. Review their protocol for confidentiality, emergency calls, communications, reporting, plus sample job descriptions and resource list.

The booklet is a publication of the Louisiana Nursing Facility Transition Grant. It is a Self Assessment Transition guide to assist individuals in nursing facilities on how to start planning for a move to the community and obtaining the supports and services they will need. It includes contact information for available state programs, along with contacts for housing options.

This application offers a total of approximately $13 million in Real Choice Systems Change (RCSC) grants to qualifying States in two grant categories: State Profile Tool (SPT): Assessing a State's Long-Term Care System & Person-Centered Planning Implementation Grant. Persons with mental illnesses are noted as target populations for both grant categories. Applications are due by July 27, 2007.

The Guardian Mentor Program was set up, in part, to assist legal guardians with issues, concerns and questions that may arise in the first year after someone from a State Center transitions to the community. This brochure explains the guardian-to-guardian mentor program which including support, information and advocacy for legal guardians.

This report describes grant activities (accomplishments and progress) in three major LTC systems areas: Access to LTC Services and Supports, Services, Supports, and Housing, Administrative and Monitoring Infrastructure. The report also provides illustrative examples of the challenges Grantees have faced as they implement grant activities, and consumers’ roles in the implementation and evaluation of these activities.

The Oregon Department of Human Services, Division of Seniors and People with Disabilities (SPD) contracted with HSRI to assist in improving the processes for assuring quality in SPD in-home waiver services. Drawing from HSRI's experience in quality assurance, SPD identified a number of states which have been particularly successful in conducting consumer surveys. At the same time, the project team explored in greater detail what was currently happening in Oregon around consumer surveys.

This paper presents the perspective that options counseling is a pivotal function of Aging and Disability Resource Centers (ADRCs) and its essential elements are counseling and decision support. The diverse array of definitions currently in use runs the risk of diminishing the importance of options counseling, which is a key function of a comprehensive long-term care system.

The purpose of this analysis is to assess the relative need for affordable housing with supportive services in Arkansas and to compare this need with the available resources for low-income senior and disabled populations. To conduct this analysis, data was gathered from both direct sources and statistical surveys. The data was analyzed to identify the gaps between needs and resources, as well as other patterns.

This study projects to 2040 the number of people ages 65 and older with disabilities and their use of long-term care services. Declining family sizes, increasing childlessness, and rising divorce rates will limit the number of family caregivers. Rising female employment rates may further reduce the availability of family care, increasing the need for paid home care. The simulations show that under the most optimistic scenario, LTC burdens on families and institutions will increase substantially.

This updated report provides the latest data on the 37 key indicators selected by the Forum for its 2004 report to portray aspects of the lives of older Americans and their families. It is divided into five subject areas: population, economics, health status, health risks and behaviors, and health care.
