Family Care Navigator: State-by-State Help for Family Caregivers

This section of the Family Caregiver Alliance website assists people in locating government, nonprofit, and private long-term care support programs in their respective areas. It includes services for family caregivers, as well as resources for older or disabled adults living at home or in a residential facility. It also includes information on government health and disability programs, legal resources, disease-specific organizations and much more.

Frameworks of Integrated Care for the Elderly: A Systematic Review

The author reviews international scholarly literature and case studies to identify four frameworks of integrated care for the elderly. The frameworks are summarized and compared based on underlying philosophy and policy, administrative and clinical best practices, and how they propose integration of different services. The review also highlights four key elements that are present in each framework and must be structured to support one another.

Aging and Disability Resource Center (ADRC) Readiness Assessment

This assessment is designed to assist organizations in evaluating their readiness to perform key functions of ADRCs. The online version provides users with immediate feedback and links to resources to help build needed capacity. Functional areas measured include: organization and governance; personnel and training; service delivery and operations; outreach and marketing; IT; partnerships; and evaluation and monitoring.

Gateway Services Consumer Satisfaction Survey & Report - Georgia

This report and sample survey were used to evaluate information and assistance services provided by GA Area Agencies on Aging (AAA). The report summarizes the responses and lists recommendations for measuring and improving consumer satisfaction with this aspect of AAA services. The Quality Assurance Team of the Division of Aging Services mailed the survey to users such as consumers and their caregivers, case managers, friends and family members. They received 840 responses.

MinnesotaHelp.info Data Management Services: RFP

The MN Board on Aging, the agency with primary responsibility for MinnesotaHelp.info, issued this RFP with the goal of contracting an organization to maintain the database underpinning the website. The site is an online directory of services helping people in Minnesota find human services, information and referral, financial assistance, and other forms of help. Proposals must be submitted by Friday, May 30, 2008.

Retooling for an Aging America: Building the Health Care Workforce

This book and corresponding materials present a strategy for improving the health care workforce serving older adults via expanded education and training, models of care, and public/private programs. The committee proposes a three-prong approach: 1)Enhance the geriatric competence of the entire workforce; 2)Increase the recruitment and retention of geriatric specialists and caregivers; 3)Improve the way care is delivered. Real Choice Systems Change and Aging and Disability Resource Center/Area Aging Grant Solicitation FY 2008

CMS has available approximately $8 million in funding to continue to support States' efforts to address complex issues in long-term care reform. In addition, CMS was also awarded $5 million for Aging and Disability Resource Center /Area Agencies on Aging grants. Applications are due no later than July 17, 2008. Identification of Consumers’ Long-term Care Needs in Illinois

This is the third in a series of reports examining various state systems and processes directed to older adults to realize the goals of the Real Choice Systems Change grants. This report analyzes the results of twenty-four focus groups consisting of 240 participants representing older adults, caregivers to older adults, and HCBS providers. In general, the results indicate Illinois has a plethora of programs and services targeting older adults, but there are disparities between communities.
