Real Choice Systems Change Grants: Compendium Seventh Edition

The Compendium contains basic information about each of the Real Choice Systems Change Grantees receiving awards in FY 2004. It includes information for the Family-to-Family and Systems Transformation Grantees who received awards in FY 2005 and FY 2006 plus the Person-Centered Planning Implementation awards in FY 2007. It is divided into sections corresponding to the different types of grants and year of award; the report helps Grantees identify others with similar goals and activities.

Aging and Disability Resource Center Pilot Project Request for Proposals - Iowa

The Iowa Dept. of Elder Affairs was seeking proposals for development of two local ADRC pilot projects to provide a coordinated system of information and access in order to minimize confusion, enhance individual choice and support informed decision-making. Core functions include planning, collaboration, long-term services, supports & options counseling. Proposals were due on 3/20/08. Use this RFP as an example in developing similar products.

The National Center for Health Statistics has announced a new release of updated statistical tests the health of older Americans showing data by age, sex, race and Hispanic origin. The tables are easy to customize. Table topics include Chronic Conditions, Functional Status and Disability, Health Care Expenditures and Utilization, Health Insurance, Health Issues, Living Arrangements, Mental Health, Mortality, Population, Risk Factors, Socio-Economic Status, Special Equipment, Medications.

Inadanna para Tinilaika - Partners for Change: Individualized Budgeting Pilot

Partners for Change funded by the Health Care Financing Administration as a Systems Change for Community Living grant, was designed to test the concept and practice of Individualized Budgeting on Guam and the application for Community-Integrated Personal Assistance Service and Support for individuals with disabilities who require such supports to live in the most integrated community settings. This report offers recommendations, interview results, references and testimonial letters.

The Administration on Aging’s Nursing Home Diversion Program

This document provides background information on the AoA’s involvement in HCBS delivery and briefly describes three of the twelve Nursing Home Diversion demonstration projects it is funding. In 2007, the AoA invited states to submit proposals to modernize both the service and system elements of their diversion efforts. This program was created in response to an Older Americans Act mandate that AoA help expand access to HCBS long-term care options.

Long-Term Care Financing: Policy Options for the Future

This report explores options for a new public-private partnership for long-term care financing. All the proposals presented try to move the partnership away from reliance on out-of-pocket financing by peo¬ple needing long-term care and their families, toward insurance, through which costs are spread across a broad population at risk of needing service, users and nonusers alike. They all rely on government rules or resources to promote that move and assume a mix of public and private financing.

When considering Medicaid’s impact on state budgets and other state spending priorities, it is important to distinguish between total spending on Medicaid and spending with state funds. Often this distinction is not made. This brief examines the different measures and provides data on how much each state spends on Medicaid.
