Palliative Care Program Outreach Materials & Forms

The Ticket to Work Palliative Care program is a demonstration project to provide palliative care interventions to help Nebraskans with disabilities address pain, symptoms and psychosocial issues that create a barrier to employment. Included are the Pilot Application Cover Letter, Consent Form and Assessment Forms. The website for Nebraska Ticket to Work is also attached.

It Doesn’t Take a Rocket Scientist to Understand & Use Social Security Work Incentives: A Manual for Social Security Work Incentives Training

This training manual provides a basic overview of Work Incentives and their parent programs. Title II Work Incentive Programs, such as Trail Work Period and Extended Period Eligibility, are discussed. Additionally, Title XVI Work Incentives, such as Blind Work Incentives and Plans for Achieving Self Support, are explained.

Medicare Part D Survey of MaineCare Workers with Disabilities Dual Eligibles: Experience Before and During Transition

In 2006, Maine’s Medicaid Infrastructure Grant was awarded funds to provide outreach about Medicare’s new prescription drug coverage to MaineCare Workers with Disabilities Option dual eligible members. Methods for raising awareness and education about the transition to Medicare Part D were implemented. To identify the usefulness of outreach efforts and to learn about experiences with transitioning prescription drug coverage, a telephone survey was conducted. A report highlights the findings.

Increasing Opportunities for Workers with Disabilities in Maine

The CHOICES CEO (Comprehensive Employment Opportunities) online newsletter, Working Together, focuses on Maine’s efforts to increase work opportunities for people with disabilities. Additionally, a brochure on MaineCare, a health insurance program for people with disabilities in Maine, includes information on benefits, qualifications, and local resources for the MaineCare option.

State Policy in Practice: A Medicaid Primer for Housing Officials

The most striking characteristic of housing and health care in this country is the disconnection between the two. Access to affordable, accessible housing is critical to the success of the Money Follows the Person Demonstration Program, which will provide opportunities for thousands of Medicaid beneficiaries living in institutions to relocate to the community. Presented in a concise question and answer format, this document explains many of the features of what can be a complicated program.

The Interaction of Policy and Enrollment in the Medicaid Buy-In Program, 2005

This report, the latest in a series on participation in the Medicaid Buy-In program, presents a snapshot of the program in 2005 that captures the interplay of policy features, enrollment trends, and participant characteristics that have made the program what it is today. Moving away from the aggregated data used in previous studies, the analysis for this report is based on individual-level data provided by states to CMS through “finder files.”

Medicaid Work Incentive Coverage for Workers with Disabilities in Hawaii

Hawaii’s MIG, entitled “Hire Abilities”, produced a draft report presenting an analysis on how Hawaii will offer the MIG program to working people with disabilities. The draft report was posted online and made available for reader feedback. The report also addresses how six other States have implemented their MIG programs.

The Caregiver Guide

The Utah Real Choice grant produced “The Caregiver Guide” and they have shared several modules. Topics covered include: Community Supports to Stay at Home, Caring for a family member with mental illness, Hiring and managing personal assistants, Assistive technology, Creating an accessible home, and Caring for a child with special needs. Modules are also available in Spanish.
