Person-Centered Planning

This promising practice profile describes the Rhode Island No Wrong Door system including key partners, the state’s long-term services and supports interagency reform initiative, and the state’s approach to person-centered options counseling. The profile includes links to important resources such as the state’s Person-Centered Options Counseling Operational Manual and the No Wrong Door System Strategic Plan.

This resource was developed for human service agency leadership whose role is directing the organization and delivery of services and supports. The five competency domains include: Strengths-Based, Culturally Informed, Whole Person-Focused; Cultivating Connections Inside the System and Out; Rights, Choice, and Control; Partnership, Teamwork, Communication, and Facilitation; and Documentation, Implementation, and Monitoring.

This resource aims to help human service agencies in states, tribes, and territories to measure their progress in developing person-centered systems. The Self-Assessment was designed to be used by staff at all levels. The Self-Assessment measures progress in eight different domains: Leadership; Person-Centered Culture; Eligibility & Service Access; Person-Centered Service Planning & Monitoring; Finance; Workforce Capacity and Capabilities; Collaboration & Partnership; and Quality & Innovation

In support of technical assistance to Idaho, the NCAPPS team prepared a Person-Centered Planning Facilitation Report which explores the experience of states that have specifically included plan facilitators in their HCBS waivers – including the circumstance under which the states deploy plan facilitators, the rates paid to facilitators, and the training requirements.

Asset Mapping is the process of mapping stakeholders and how they are engaged by an agency and/or its allies. NCAPPS partners at Collective Insight, LLC, created a comprehensive Asset Mapping Toolkit to help organization looking to better understand and deepen their connections with the communities they serve.

The National Quality Forum (NQF) has published the final report from the Person-Centered Planning and Practice Project. This report outlines recommendations from a committee of stakeholders with lived and professional experience in LTSS. The report includes the committee's final recommendations across a number of areas regarding person-centered thinking, planning, and practice.

This issue brief discusses the pressing legal needs of older adults and the ways that coordinating health care, legal, and social services can improve their well-being. The brief examines three medical-legal partnership programs serving older adults and explores their impact, including the prevention of homelessness and the improvement of financial stability.

The white paper examines the impact that managed care organizations can have on cost and quality of care for older adults and people with disabilities. The National Council on Disability found that MCOs can lower the costs of services for these populations. However, MCOs must face the challenge of training staff on person-centered planning and competency for the quality of care to genuinely increase. This provides an analysis of the positives and negatives of MCOs and where to move from here.
