Section 1915(i) HCBS State Plan Option

Health Homes

CMS has approved the District of Columbia’s Health Home Planning Request. As of June 2014, the District of Columbia has not officially submitted a Proposed Health Home State Plan Amendment to CMS, but the District of Columbia plans to participate in the Health Home State Plan Option in FY 2014.
