This guidebook is written to provide information about Minnesota’s Personal Care Assistance (PCA) program and the many options consumers have. The state strives for a consumer directed service model. Sections in this guidebook define services, help consumers develop skills and provide resources to help take more control of the important services that allow independent living. Passport to Independence - Delaware’s Nursing Home Transition Program

Passport to Independence is a federally-funded pilot project led by Delaware Health and Social Services’ Division of Services for Aging and Adults with Physical Disabilities (DSAAPD). The overall goal of the program is to identify, inform and assist nursing home residents, especially those who are Medicaid-eligible, who want to move to a new home by offering individualized case management to accomplish the goal. Quality Waiver Review Database

This database, developed under Maine's QA/QI grant, helps to organize program design and QM system information in a way that is compatible with both CMS's Quality Framework and Review Protocol. Once information is entered into the system, the database can generate one page snapshots of a state's QM structure. <br> Please Note: You must download either database to your computer in order to run.

Comprehensive Assessment Reporting Evaluation (CARE) tool

Washington’s automated, comprehensive, person centered assessment and planning tool measures the personal care needs of anyone receiving services at home or in a residential setting and develops a care plan. The tool contains 60 different screens, some relate to risk. Participant selects a service setting and then the program computes eligibility, need and a service plan that crosses settings. The package contains: CARES Training Manual, CARES Assessment, Eligibility & Rates for LTC Services.

Individual Support Plan System

Developed by Oregon’s Office of Home & Community these tools include consideration of 43 areas of risk and illustrates the state’s risk tracking documentation in Q&A format. They also provide instruction to staff with loops back to participant preference and guidance to protocols to address identified risks. Forms included: ISP Addendum, Financial & Individual Support Plan, Personal Focus Worksheet, Risk Training, Protocol for Aspiration, Constipation, Dehydration, Seizure and Signature Sheet.

Handbook for Transition Navigators

This handbook, drafted by Jane Schiele and edited and revised by Laura Hershey (HCPF Systems Change Section), is designed to help other advocates or agencies to inform many more individuals about their right to live in the community, and to assist those who desire a transition back to their community. The report defines transition and offers the nuts and bolts of the program and lastly overs tips for moving day and beyond.

Aging and Disability Resource Center (ADRC) Business Plan

This document provides an overview of essential elements of a non-profit business plan for the ADRC. Sections II and III are written to serve as a template that can be easily adapted by each ADRC. This should only require filling in information in certain sections and modifying text to better fit your effort. Sections IV through VIII contain discussions and tools to complete each of these sections. They will require more effort and original writing to complete.
