Issue Paper: Core set of Consumers’ Personal-Experience Outcomes and Assessment

Part of the Wisconsin QA/QI grant was to develop a framework to evaluate long-term care services. The framework was based on desired personal-experience outcomes. Once desirable outcomes were recorded, they were used as benchmarks for quality management and outcomes measurement.

Quality Close to Home: A Preliminary Design for an Integrated Quality Management System

The “Quality Close to Home” papers are part of Wisconsin’s process to review the existing quality management system for Home and Community Based Services. The report on local practices was the result of a series of interviews conducted with local informants who conduct quality management activities. The second paper outlines the scope of quality management, challenges of quality management, federal rules and regulations, and provides a design for an improved quality management system.

The Caregiver Guide

The Utah Real Choice grant produced “The Caregiver Guide” and they have shared several modules. Topics covered include: Community Supports to Stay at Home, Caring for a family member with mental illness, Hiring and managing personal assistants, Assistive technology, Creating an accessible home, and Caring for a child with special needs. Modules are also available in Spanish.

Measuring Quality Using Experience Surveys & Results

The following two reports are the result of face-to-face and mailing surveys to evaluate user experiences. The face-to-face surveys were conducted with people who are 18 years of age and older who receive Medicaid waiver or Intermediate Care Facilities for Persons with Mental Retardation (ICF/MR) Program Services. The Children/Family Surveys were mailed to families of children under 18 years of age who received services in programs operated by the Department in 2005.

Quality Assurance and Improvement: DataMart Software Requirement Specifications and Reports

The goal of the QAI Datamart is to develop a consumer-based reporting and data analysis database using the Home and Community-Based Services Quality Framework. The QAI DataMart will support data analysis and decision support for quality assurance and improvement. The purpose of this Software Requirements Specification (SRS) is to document the customer’s requirements for the applications being developed or enhanced. The reports and queries available from the DataMart are also included.

Critical Incident Reporting and Recommendations

One objective of the Texas’s QA/QI grant was to develop a uniform and automated critical incident reporting process. Information was collected on critical incidents reported most often by other states to be used with the Quality Assurance and Improvement (QAI) Data Mart. The QA/QI task force reviewed and used this information to develop a list of critical incident definitions and recommendations.

Coordinating and Leveraging Long-Term Supports with Affordable and Accessible Housing for Persons with Disability: Maryland Case Study

This case study describes an approach to public and private sector agencies coordinating and leveraging resources at a state and local level to expand affordable and accessible housing options for persons with developmental disabilities. Homes for America utilized low-income housing tax credits to set aside six units of housing in an affordable multifamily development to be converted to ownership at the end of a fifteen-year lease period for Medicaid beneficiaries.

Coordinating and Leveraging Long-Term Supports with Licensed Affordable Assisted Living: Arkansas Case Study

This case study describes how a private sector entity, NCB Development Corporation, helped to coordinate and galvanize the existing efforts of essential partners to develop, plan, and finance a new assisted living facility, and coordinate appropriate funding sources for operational and client service expenditures. The Gardens at Osage Terrace was developed under the Coming Home Program, a demonstration program of NCB Development Corporation and the Robert Wood Johnson Foundation.

Coordinating and Leveraging Long-Term Supports with Affordable Assisted Living in Public Housing: Florida Case Study

This case study describes the development and operations of assisted living units coordinated with independent senior units in a public housing location: Titusville Towers in Titusville, Florida. Because this example uses an existing facility, replication of this case study demonstrates that time and cost will be substantially less than new construction of an assisted living facility. The Titusville Housing Authority received approval from HUD to designate it as seniors-only housing.

Coordinating and Leveraging Long-Term Supports to Live More Independently with Assistive Technology and Home Modifications: Nebraska Case Study

This case study describes the leveraging and coordinating of multiple funding resources to provide increased access to home modifications and assistive technology in response to an assessment of individualized need. The Nebraska Assistive Technology Project serves as a single provider to conduct program eligibility for all funders, and provide assessment, determine need, and develop a service plan for consumers.
