The Colorado Dept. of Health Care Policy & Financing (HCPF) developed a program that enables people with disabilities to manage their own attendant services. In the CDAS, people hire, train, supervise and fire their own attendants. They set their own attendant schedules and to a degree they determine what services the attendants provide. HCPF is not accepting applications for the CDAS at this time however, the forms provide an overview of the program and serve as templates for similar programs.

The purpose of the New York Real Choice Training Manual is to provide formal training to self-advocates who wish to become peer mentors. Mentor trainees will learn through the adult education model, which builds on the strength, capacity and experience of the learners. Self-Advocate consultants/mentors bring with them an incredible wealth of life experiences, which prepare them for this role.

This report focuses on individuals with Alzheimer's and other dementias. The project instituted a comprehensive system to support enduring changes in the delivery of community-based assistance for Medicaid eligible persons with dementia. The projects two goals: facilitate hospital-to-home transitions for individuals wishing to stay in the community; and field test a nursing home-to-community transition program for eligible individuals. The workbook used in discharge planner training is included.

This policy outlines the role and function of the Delaware Health and Social Services, Division of Developmental Disabilities Services Performance Analysis Committee. This group plays a central part in the collection, analysis and reporting of data in relation to performance indicators identified by various stakeholders as important in a state MR/DD program. The policy offers definitions, standards, procedures, and a flow chart to illustrates their continuous quality improvement cycle.

Given the complexities of F/EA operations, it is important to refer to reliable source information such as the lists of Government F/EA and Vendor F/EA Tasks. This information will assist state and local program agency staff in understanding the requirements and key elements of Government and Vendor F/EA operations and developing a Government F/EA Policies and Procedures Manual, when applicable.

In light of this major change and assuming that states may want to learn more about Cash & Counseling, this publication offers an in-depth description of the option, a discussion of key findings from the three-state demonstration, and preliminary information about the program’s expansion into 12 more states.
