Report describing methodology and results of Oregon Statewide Survey of Mental Health Drop-In Centers. Survey was commissioned by Oregon Real Choice grantee as part of its efforts to focus on the lack of meaningful choice and self-determination for consumers in the mental health network.

The attached files are the instructions for filling out the Alaska Consumer Assessment Tool (CAT) and a copy of the tool. The form was designed to be an objective tool that is easily coded. Each subsection of the manual includes the “how to complete” instructions and the time frames in which to measure the person’s abilities. The design facilitates immediate eligibility determination by the assessor.

According to program staff, VNS CHOICE has reduced hospital and nursing home use among participants and has low disenrollment.

This document highlights the Village Integrated Service Agency, a program of the Mental Health Association in Los Angeles County. The program provides coordinated, comprehensive services for people with mental illness. Available services are determined by the needs and goals of the participants. This report describes the program and its effectiveness in helping people with mental illness live independently.

The Utah Family Council is a non-profit organization that provides support to all families of people with disabilities, regardless of whether they receive public funding. This document describes this program, which uses funding from the Self-Determination Grant from the Robert Wood Johnson Foundation to increase availability. This report briefly describes the New Jersey Statewide Respite Care Program. The New Jersey Department of Health and Senior Services (DHSS) administers a state-funded, Statewide Respite Care Program providing a wide variety of services to relieve unpaid, informal caregivers from the stress of caring for older people and/or people with disabilities. The respite program customizes services to the person and the caregiver, with a focus on alleviating caregiver stress.

The Formative Evaluation of the Real Choice Systems Change Grants evaluates how the Systems Change grantees are progressing towards meeting their goals. The Formative Evaluation identifies problems faced by the grantees and ways that they deal with the problems. This document provides summary information about each of the FY2003 Grantees.

In fiscal year (FY) 2004, Congress appropriated approximately $40 million in funding for a new round of Real Choice Systems Change Grants. With this solicitation, the Centers for Medicare & Medicaid Services (CMS) invites proposals for grants totaling approximately $31 million of these funds.
