Consumer Centered Quality Assurance and Quality Improvement in Home and Community Based Services RFP

The Delaware Health and Social Services (DHSS), Division of Developmental Disabilities Services (DDDS), requested proposals to select a contractor to provide consultation in the area of Quality Assurance/Quality Improvement (QA/QI) in Home and Community Based Services. The Request for Proposals was numbered NO. PSC617 and due by 08/13/04. Funds were available under a grant from the Centers for Medicare and Medical Services (CMS).

The Cross Cultural Health Care Program Website

Recognizing the diversity and the different ways to health, the mission of the Cross Cultural Health Care Program is to serve as a bridge between communities and health care institutions to ensure full access to quality health care that is culturally and linguistically appropriate. Through a combination of cultural competency trainings, interpreter trainings, research projects, community coalition building, and other services, the CCHCP serves as a bridge.

FY2004 Real Choice Systems Change Grants: Award Announcement

On October 5, 2004, HHS Secretary Tommy Thompson announced $31 million in grants to 31 states to help people with disabilities or long-term illnesses live in their homes and participate fully in community life. The grants were awarded under the New freedom Initiative and are known as Real Choice Systems Change Grants. A total of 52 grants were awarded in nine categories.
