This report outlines an integrated strategy to update the country’s long-term care services infrastructure in preparation for the aging of the population over the next thirty years. Recommendations are focused on the areas of quality, workforce, technology, and financing.

The indicators provided in this fact sheet serve as a means for self-assessment by employment programs to help make strides toward facilitating competitive and sustainable employment choices for people with disabilities. This tool can also be used to identify both areas of strength to highlight in marketing and areas that need priority attention for improvement.

This study examines dynamics affecting the direct-care workforce in Vermont. It found that only one third of those workers have employer-sponsored health insurance, one half of those surveyed expected to receive a raise, and 42% received formal job training. The report includes nine recommendations to strengthen the state’s direct-care workforce. In addition, the appendices include original surveys given to employers and direct-care workers.

Consumer direction of personal assistance services (CD-PAS) is one model of service delivery that gives Medicaid beneficiaries varying degrees of control over hiring, scheduling, training, and paying personal care attendants. Program administrators, experienced with the programmatic features of CD-PAS in their state, were interviewed. Eligibility criteria, participant support services, and method of financial management used by Medicaid beneficiaries to pay direct care workers are discussed.

The Compendium contains basic information about each of the Real Choice Systems Change Grantees receiving awards in FY 2004. It includes information for the Family-to-Family and Systems Transformation Grantees who received awards in FY 2005 and FY 2006 plus the Person-Centered Planning Implementation awards in FY 2007. It is divided into sections corresponding to the different types of grants and year of award; the report helps Grantees identify others with similar goals and activities.

The term Quality Councils refer to stakeholders (including representatives from the state agency, family members, providers, and consumers) who convene to address improvement in the quality of services delivered to individuals with developmental disabilities and their families. Find 10 surveys examples used to assess provider services at 1st Tennessee ADRC pilot site. Surveys contain questions to obtain consumer feedback on a variety of services, including transportation, legal services, meals, ombudsman, service coordination, personal care and homemaker services.

Following the Gulf Coast Hurricane disasters in 2005, the NASDDDS Emergency Response Preparedness Self Assessment Instrument was developed to assist state developmental disabilities officials evaluate the extent to which their existing preparedness plans address the unique characteristics and needs of people with intellectual and developmental disabilities during periods of emergency.
