Redesigning Long-Term Care Systems Through Integrated Information Systems

The National Aging Services Network has played a key role in integrating information about community services, including health, social support, educational, and recreational services, resources, and funding streams. This document compares two Area Agency on Aging (AAA) computerized information systems in Georgia and Indiana. This study is one of a series of five reports called Case Studies from the Aging Network: Health and Social Support Systems Integration.

Point of Entry Systems for Long-Term Care: State Case Studies

This report was prepared to inform the New York City Department for the Aging about other states’ long-term care (“LTC”) point of entry (“POE”) systems. Needs Assessment for Children with Special Needs - Nevada

The Nevada Children with Special Health Care Needs conducted a 5 month multi-method needs assessment that describes demographics, universal needs and barriers families with children with special health care needs face. Completion of this needs assessment represents an opportunity for community input into the prioritization of needs design & implementation of positive changes to the system of care. Aging and Disability Resource Center Guidance for Evaluation Plan

This document provides information on elements to include in the ADRC evaluation plan. This resource will provide guidance on what to include, and will serve as an example of how to organize the elements into a matrix, but this format can be tailored by each grantee. Elements to incorporate into the ADRC evaluation plan include: Project Objectives; Tasks; Indicators; Data Sources; Data Collection Methods; Timeline/Interval; Responsible Entity; and Instruments.

Promoting Older Adult Health: Aging Network Partnerships to Address Medication, Alcohol and Mental Health Problems

This guide profiles 15 successful programs that have partnered with community organizations to address medication, alcohol and mental health problems in older adults by promoting new linkages between well-known, trusted, and heavily utilized providers of aging services and relevant substance abuse and mental health services. By joining these systems, we can more successfully identify older adults who are at risk and connect them with the prevention, education, outreach, and treatment services.

Issue Brief on Prevention: Considering Prevention in the Development of an Aging and Disability Resource Center

This article profiles some suggests on prevention which occurs on three levels. Primary prevention targets healthy individuals and groups to stop disease before it starts; secondary prevention targets those at early stages of disease or with high risk behaviors but who are asymptomatic; and tertiary prevention targets those with symptomatic disease, to manage or improve their conditions. ADRCs can promote health by offering information assistance and resources on each of these levels.

Promoting Young Children\'s Healthy Mental Development: News Brief

This brief summarizes presentations made at NASHP’s 17th annual state health policy conference during a session on “Promoting Young Children’s Healthy Mental Development.” The session focused on the evidence base, state program highlights, a discussion of the ABCD program, and ways to use the Medicaid program to better support early childhood social and emotional development.
