RTI - Systems Change Evaluation

This paper describes the activities of the 19 Grantees funded in fiscal year 03 and 04 from CMS and six Grantees funded in fiscal year 02 by the Maternal and Child Health Bureau (MCHB). F2F Health Information Centers assist families of children with special health care needs. This paper provides information states and stakeholders will find useful when planning or implementing similar initiatives. The Compendium contains basic information about each of the Real Choice Systems Change Grantees. The Compendium will help you learn more about how these grants will be used to allow more people of all ages with a disability or long term illness to live and participate in their communities. Real Choice Systems Change Grantees will also find the Compendium useful to identify other Grantees with similar goals and activities. The fifth edition of the Compendium replaces all previous editions.

This report describes the FY 2002 and FY 2003 Grantees’ challenges and progress using information provided by the Grantees during the reporting period October 1, 2003 to September 30, 2004. The report describes grant activities in four major LTC systems areas: Access to Long-Term Care Services and Supports, Services, Supports, and Housing, Administrative and Monitoring Infrastructure and Long-Term Care Service and Support Workforce

This report provides an overview of the NFT initiatives implemented by 18 of the FY ‘01 and FY ‘02 Systems Change Grantees. It describes their differing approaches to nursing facility diversion and/or transition within a framework of the key steps needed to create programs that are integrated into a state’s LTC system. This report also identifies the transition challenges and policy issues states and ILCs are facing, and discusses the implications for moving forward with rebalancing efforts.

The Formative Evaluation of the Real Choice Systems Change Grants evaluates how the Systems Change grantees are progressing towards meeting their goals. The Formative Evaluation identifies problems faced by the grantees and ways that they deal with the problems. This document provides summary information about each of the FY2004 Grantees.

This report provides a summary of the partnership development activities undertaken by the 52 Grantees that were awarded a Systems Change Grant in 2001. It describes the ways that Grantees involved consumers and public and private partners in the development of Grant applications and their plans for involving them in implementation activities. In addition, the report includes a more in-depth description of partnership involvement for nine Grantees (three from each Grant type).

The purpose of this report is to compare and contrast the fiscal year (FY) 2001 Systems Change Grantees’ goals and planned activities as identified in their Grant applications submitted to CMS. This report provides a summary of the goals of the 52 FY 2001 Grantees and representative examples of the various activities they plan to implement to help them achieve these goals. The report also includes a summary of States’ views of the strengths, challenges and barriers of their LTC systems.
