Promising Practices in HCBS: Wisconsin- Supported Housing for People with Disabilities

This report describes the State of Wisconsin’s program to assist people with disabilities find housing. The state employs a supported housing specialist who works with local communities to help people with disabilities and their families to find affordable housing solutions. In addition, the supported housing specialist works with individuals with disabilities, local agencies, developers, housing providers, lenders,and other stakeholders to build relationships and improve housing access.

Promising Practices in HCBS: Indiana- Coalition of Organizations with Resources about Home Ownership and Rental

This report focuses on the Back Home in Indiana Alliance project. This project facilitates home ownership and rental by connecting people with disabilities and their families to a variety of housing organizations and resources. These organizations and resources include lending programs for low-income or under-served people, down payment assistance, grants for home modifications, and programs that link apartment seekers with appropriate, low-income rentals.

Promising Practices in HCBS: New York/Albany-HUD Financial Operations Center Partnership with Northeast Career Planning Under Javits-Wagner-O\'Day Program

This report looks at the Javits-Wagner-O’Day program, which employs people with disabilities to provide services for the federal government, through partnerships with local vocational support agencies. In the spring of 1998, the U.S Department of Housing and Urban Development (HUD) Albany Financial Operations Center entered into such a contract with Northeast Career Planning (NCP), a non-profit, vocational rehabilitation organization. This report describes the implementation of that program.

Promising Practices in HCBS: Oregon- Maximizing Participant Control Over Services

Oregon's Independent Choices Program allows Medicaid-eligible individuals to pay cash directly to providers for personal care and related services. Under this five-year project, participants receive a monthly cash payment and are fully responsible for the mechanics of payroll and budgeting for needed services.

Promising Practices in HCBS: Florida- Cash Allowances and Support Services for People with Disabilities
