Utah Care Notebook

This Care Notebook is an organizing tool for families who have children with special health care needs. Utah created the book to help families keep track of important health care information, list providers and community organizations contact information, prepare for appointments and file and share health history. Utah Family Voices shared this guide so that other resource centers may use this as a template when developing a similar tool.

Did You Know, Now You Know training

Provided by Family Voices of Wisconsin, this training is organized into two modules and designed to provide a comprehensive, primary overview of health care and community supports for children and youth. The overall goal of this initiative is to help families access services that will benefit their children. The goal is that one day, they won't hear parents say, \"Why didn't anybody tell me about this before?!\" Presentation slides and evaluations are included.

Family-to-Family Health Care Information & Education Center brochure: Massachusetts

Review an example of how grantees can create informational brochure for families of children and youth with special health care needs. The outreach material explains the centers benefits including training, mentors, advocacy and listserv services. There are three versions, one in English, Portuguese, and Spanish.

Community Resources Poster and Cards: Indiana

Family to Family Grantee, the Indiana Parent Information Network, developed and shared two of their outreach tools. The poster and community resource cards are to be placed at physician’s offices and clinics. The posters were laminated so that local numbers could be added with dry erase. The cards also allow space to individualize the community resources being referred.

2007 Community Alternatives to Psychiatric Residential Treatment Facilities (PRTF) Demonstration Grant Announcement

CMS announced $218 million in demonstration grants awarded up to 10 States to help provide community alternatives to psychiatric residential treatment facilities for children. These demonstration grants enacted by the Deficit Reduction Act, available over a five year period, will also assist States in adopting strategic approaches for improving quality as they work to improve each child's functional level in the community. Applicants' teleconference Sept. 19 & proposals are due Oct. 18, 2006.

Maryland Medicaid Home and Community-Based Long Term Care Services Booklet

This guide describes Medicaid services that are designed to help individuals who are at risk of institutionalization remain in their homes. It describes the range of home and community-based services available through Medicaid and can help consumers, families, and health care professionals make informed decisions about long-term care services. This brochure was developed as part of their Real Choice grant in 2005 where over 7.500 copies were distributed. Review their latest 2006 version.

Sustainability: Sustaining a Nonprofit Organization

Throughout the development process, programs must consider the steps to sustainability including people, planning and preparation, publicity/public relations, facilities, programs, policies and procedures, and persistence and positive passion. This entry includes a paper by Florene Stewart Poyadue offered to Family to Family grantees during a technical assistance call, plus the transcripts from the session where participants discussed the 7Ps of sustainability.

Incorporating Youth Development Principles into Adolescent Health Programs: A Guide for State-Level Practitioners and Policy Makers

Practitioners and policy makers who focus on youth must be knowledgeable about youth development concepts, but it is critically important they know how to apply those concepts in the context of specific functions and program responsibilities. The emphasis of this paper is on incorporating youth development into state adolescent health programs however, the information is relevant to other state-funded programs that address various adolescent issues.

Real Choice Systems Change Grant Program Fourth Year Report: Progress and Challenges of the FY 2003 and FY 2004 Grantees: (Oct. 1, 2004 – Sept. 30, 2005)

This report describes grant activities in three major LTC systems areas: Access to Long-Term Care Services and Supports, Services, Supports, and Housing, Administrative and Monitoring Infrastructure. For each of the focus areas the report describes Grantees’ accomplishments, illustrates the challenges and discusses the consumers’ role in the implementation and evaluation of activities. The report uses information provided during the reporting period October 1, 2004 to September 30, 2005.
