New Credential: Certified Professional in Age-Friendly Health Care (CPAFH)

The Institute for Healthcare Improvement (IHI) has developed the Certified Professional in Age-Friendly Health Care (CPAFH) credential. This professional certification program affirms a candidate's ability to provide safe, person-centered care across settings, and is informed by the evidence-based 4Ms Framework (What Matters, Medication, Mentation, and Mobility). The CPAFH certification is backed by leaders at the John A. Hartford Foundation (JAHF), American Hospital Association (AHA), and the Catholic Health Association of the United States (CHA). The certification covers areas like geriatric syndromes, medication management, care transitions, and culturally responsive communication. Candidates for this certification can include physicians, nurses, social workers, case managers, care coordinators, rehabilitation professionals, and health system leaders.
